• Before your Appointment:
    • A COVID questionnaire will be emailed or texted to you
    • You must fill out the questionnaire and all requested forms before arrival or day of appointment
    • Pre-select 4-5 eyeglass frame options/interests here (TBD). Text, email or bring the results
    • Take a quick quiz on your lens options and needs. Text, email or bring the results
    • Obtain and wear a face mask. If unable to obtain a mask, a disposable mask will be provided
    • Text or email images the front and back of new insurance cards

  • Day of Your Appointment:
    • Wear your contact lenses into the office
    • Bring your most recent pair of eyeglasses and sunglasses
    • Arrive 15 minutes early
    • Text or call the office and wait in your car to be admitted through the front door
    • A technician will call you to take your health history before entering the building
    • Companions should remain outside the office unless they are a parent, guardian or caregiver. One necessary companion will be allowed in. All companions will take the COVID questionnaire and have their temperature checked
  • During Your Appointment:
    • Your temperature will be taken at the door with a non-contact thermometer
    • Continue to practice good hygiene:
      • Wash your hands with soap and water or use hand sanitizer, especially after touching frequently used items or surfaces
      • Avoid touching your face
      • Sneeze or cough into a tissue, or the inside of your elbow
      • Disinfect frequently used items and surfaces as much as possible
      • Strongly consider using face coverings while in public

~WARNING~

  • All Vulnerable Individuals should continue to shelter in place
    • On a medical need basis only, vulnerable individuals will have priority time slots at the beginning of each day
  • Members of households with vulnerable residents should be aware that by returning to work or other environments, they could carry the virus back home. Precautions should be taken to isolate yourself for vulnerable household individuals or isolate yourself from vulnerable residents if leaving the home
    • Vulnerable Individuals Include:
      • The elderly (>65yo)
      • Individuals with serious underlying health conditions are those with:
        • High blood pressure
        • Chronic lung disease
        • Diabetes
        • Obesity
        • Asthma
        • Compromised immune system such as by chemotherapy for cancer and other conditions requiring such therapy
  • People who feel sick should stay at home
    • Do not go to work, school, or other settings
    • Contact and follow the advice of your medical provider
